- The Affordable Care Act's Health Insurance Marketplace coverage became available on January 1, 2014, and its Federally-financed, state-optional, Medicaid expansion started on the same day. Since then, millions of Americans have gained coverage through each of these sources, and total insurance coverage has expanded markedly.
