- Abstract:
- KEY FEATURES. Patients with complex, unmet care needs receive hands-on support from primary care teams coached through videoconference sessions led by experts in addiction medicine, psychiatry, and other fields. TARGET POPULATION. Low-income Medicaid beneficiaries with serious mental illness, substance abuse, and other challenges--an underserved population that accounts for a disproportionate share of health spending in the U.S. WHY IT'S IMPORTANT. Current care models and capacity are not adequate to meet the needs of patients with confounding behavioral health and social support needs. BENEFITS. Early results show significant drops in emergency department and hospital use, with reduced costs to Medicaid. CHALLENGES. Finding and retaining primary care practitioners who are willing to work in inner-city and remote areas.
